Applications for Child Care Centers
INSTRUCTIONS TO APPLY
1. Download and complete all pages of the application packet.
2. Attach a copy of recent income documentation:
a) Recent (within last 30 days) paycheck stub OR
b) Recent (within last 30 days) statement (within last 30 days) from the director/owner stating the applicant’s hourly wage and the number of hours worked per week. Statement must be on center letterhead and dated/signed by the director or owner.
3. Make sure all forms are signed and dated where indicated.
4. Mail application forms and income documentation to the state T.E.A.C.H. office:

5. You will be notified if further information is needed.
6. Once approved for the scholarship, you will be sent an email from your assigned T.E.A.C.H. counselor to schedule a welcome call. You will then receive further instructions.
